Any athlete that had a physical exam this year only needs to complete Form B for the 2020-21 school year. This may be uploaded on rschool instead of a physical form from a doctor. The form must be signed by both parent and athlete before uploading. Please note that if the athlete answers any of the questions ‘yes’, they will need a clearance note from a doctor. In this case, the athlete IS NOT cleared for practice or game activity, even though the form may be uploaded. Clearance notes may be uploaded as a separate page with form B when completing the rschool registration.
